Overview.

BCR-ABL is a fusion gene abnormality found on chromosome 22 of leukaemia cancer cells, which is the cause of chronic myelogenous leukaemia (CML) and some other leukaemias.

The BCR-ABL minor transcript is a fusion gene typically associated with Philadelphia chromosome-positive (Ph+) acute lymphoblastic leukaemia (ALL). It results from a translocation between chromosomes 9 and 22 [t(9;22)(q34;q11)]. This leads to the production of a 190 kDa (p190) fusion protein, distinct from the p210 protein commonly found in chronic myeloid leukaemia (CML). Detection of the BCR-ABL minor transcript by RT-PCR is crucial for diagnosing Ph+ ALL, guiding targeted therapy (such as with tyrosine kinase inhibitors like imatinib or dasatinib), and monitoring minimal residual disease (MRD). Early and accurate detection significantly impacts prognosis, as Ph+ ALL typically has a more aggressive course but responds well to targeted treatments when identified promptly.
